Start from a template

Loads a starter set of fields for that kind of form. Toggle any off and edit the rest from there.

Save responses to a new Google Sheet The generated script creates a spreadsheet and connects it to the form automatically. Turn this off to leave responses in the form's own Responses tab.

Collect the respondent's email Google asks each person for their email address when they submit.

Limit to one response per person Respondents have to sign in to a Google account to submit. Leave off if people should be able to submit without signing in.

Confirmation message (the thank-you shown after someone submits — optional)

Create the Google Form

Copy the script — or email it straight to whoever runs your Google account: Copied Opening your email…
Open script.google.com/create while signed in to the Google account you want the form on.
Delete the placeholder code, paste in the copied script.
Click Run (▷). The first time, approve the permission prompt — it's your own script acting on your own account.
Open View → Logs to get the edit link, share link, and (if you turned it on) the link to the connected response sheet.
